What are the 3 parts of a dental implant?

The 3 Main Parts of a Dental Implant

1. The Fixture

The fixture is the bottom of the implant and is the part that is embedded below the gum line, fusing with the jawbone.  It is like the root of the tooth. The fixture is made of titanium and has a cylindrical, screw-shaped design. It is often coated with a material known as hydroxyapatite, which can assist with the process of fusing the implant to the jaw bone. This part is placed first and appropriate healing has to take place before the restorative phase, which includes placing the abutment and crown.

2. The Abutment

The abutment is screwed into the fixture and will eventually hold the actual tooth portion that you see in your mouth. It is a small piece that lies right around the gum line.

3. Implant crown

The crown is cemented or screwed into the abutment. If the prosthesis is a crown or bridge, or snapped onto the abutment if the prosthesis is a bridge or denture.

Fixed Bridge

A bridge is an option when there are teeth on either side of the missing tooth.

Advantages
It stays cemented in the mouth and does not come out.

Disadvantages
This does involve cutting down the teeth on either side of the missing tooth to be able to connect them together. Floss threaders, which slide under the bridge, should be used daily, to keep it clean.
The disadvantages for bridges are cutting down teeth that might be perfectly healthy. If there is a leak under the anchor teeth of the bridge, decay can occur below. A bridge can replace one or two missing teeth easily, and sometimes more if there is enough support.

Removable partial denture

A removable partial denture is worn during the day. This is used to replace one or many missing teeth.

Disadvantages 
-Sometimes may need metal clasps to help it stay in the mouth which may be visible when speaking or smiling.
- They move a little when speaking or eating, which can be uncomfortable.
-We recommend taking them out at night and soaking them in a cleaning solution.

Advantages
There is no need to file down any teeth, and this option is usually the most economical. A full denture can replace all missing teeth.

Dental Implant

An implant is a great way to replace a missing tooth.

Advantages
This solution feels and acts the most like a natural tooth when chewing, brushing or smiling. A major advantage is no need to alter any other teeth when placing an implant. An implant is a great solution for a single missing tooth, and even more implants can replace many missing teeth. When smiling or talking there is no way to tell the difference between an implant and a natural tooth.

Disadvantages
The implant replaces the actual root of the missing tooth and heals in the bone for 3 to 7 months and we then begin the restorative phase. We or your dentist then place an abutment, to which a crown will be cemented. Dental implants are usually a more costly option, at least initially, but are often the best long-term investment.

Do Nothing

There is always the option to do nothing.
Advantages
Saves money, at least initially
Disadvantages
When a tooth is removed, the bone shrinks away and the other teeth in the area will shift to fill the gap. This can sometimes lead to other problems like shifting of other teeth, causing bone loss or decay around the existing teeth as they become more difficult to clean.

Gum recession is the process in which the gum tissue that which surrounds your teeth wears away, or pulls back, exposing the tooth's root. If left untreated, gum recession may ultimately result in tooth loss.

Most people don't notice their gums receding because it occurs very gradually. You may notice tooth sensitivity, or that your tooth looks longer than normal. Sometimes a notch can be felt near the gum line with your fingernail
Gum recession is often caused by periodontal disease. If untreated, gum disease leads to the destruction gum tissue and supporting bone that hold your teeth in place.

Aggressive tooth brushing. If you brush your teeth too hard, it can cause the enamel on your teeth to wear away and your gums to recede.  Clenching and grinding of the teeth can also contribute to gum recession and defects in the tooth roots. Many people do not even realize they have this habit, and many do it in their sleep!

Insufficient dental care. Inadequate brushing, flossing, and rinsing with antibacterial mouthwash makes it easy for plaque to harden into tartar or calculus, a hard substance that builds on and between your teeth and can only be removed by a professional dental cleaning.

Concentrated blood has been used in hospitals for a long time to accelerate healing.  Leukocyte Rich Platelet Rich Fibrin (L- PRF) is a by-product of a patient’s own blood that can help healing after dental procedures such as a tooth extraction, as well as promote healing around bone or tissue grafts and dental implants.

We are seeing more and more advertisements for “affordable” dentures. What does “affordable” or "discount denture" mean? Are these dentures the same thing that for a lower, fairer price? Unfortunately, the answer is NO!

There is a big difference in the outcome of customized dentures and "discount" dentures. Here are some of the main differences:

1) Quality dentures take TIME. Multiple visits are necessary to create a totally customized, well-fitting dentures. Beware of advertisements for dentures which can be made in a day. Discount denture centers often tell patients they can get their dentures the “same-day”, but most likely the result will be poor due to a lack of customization. Quality dentures must be tried in to check the bite, fit, and appearance of the teeth (and approved by the patient) before they are finalized. Customized dentures may take more time, but this time in the creation process results in less adjustments, a better looking denture, AND a happier patient.

2) It is wise to use the very best materials from start to finish when creating a denture. Quality dentures cannot be made with cheap materials or by cutting corners. Expert dental lab technicians should be involved in fabricating dentures along with the use of quality materials. This can mean the difference between a denture that looks natural versus one that looks fake, as well as how they fit and how long they last. Many “affordable” or discount dentures broken after just a few months due to the use of cheaper and less durable materials. As the saying goes: You get what you pay for.

3) Also beware, most discount denture centers do not back up their product with quality service and will charge for each and every adjustment, which quickly brings their cost up. Many patients are forced to choose not to have adjustments to their discount dentures because of the price, and they must put up with horrible pain or just leave their dentures in a drawer and go without teeth.

4) Lower dentures can be very difficult to wear. Lower denture treatment discussions, when possible, should include the option of at least two lower dental implants. Not everyone will choose implants due to cost, but everyone deserves to have this option presented. Often in an "affordable denture" situation this is not even offered and the difficulties of wearing a lower denture are not fully explained.

5) Dentures need regular maintenance such as relines from time to time. Customized dentures are easier to maintain, reline, repair and adjust due to the better quality of their material.

If a patient has severe bone loss surrounding existing teeth, Dr. Britten may recommend a regenerative procedure called Periodontal Regeneration Grafting. This procedures can reverse some of the damage by regenerating lost bone and tissue.

During this procedure, Dr. Britten accesses the deeper disease and removes the disease‐causing bacteria in a procedure called osseous surgery . Barrier membranes, bone grafts or tissue‐stimulating proteins can then be used to encourage your body’s natural ability to regenerate bone and tissue. First, the extraction of a root tip: Careful management of extraction sockets after a tooth is removed prevents unsightly bone loss and provides a better cosmetic outcome for tooth replacement.

This is because your body perceives that once the root is removed, the bone is no longer needed and your body absorbs or removes it, creating a ridge deformity.

Gingival (gum) recession: Its causes, side effects, and possible treatment options, gingival (gum tissue) grafting.

When recession of the gum tissue occurs, the body loses a natural defense against both bacterial penetration and trauma. When gum recession is a problem, gum reconstruction using grafting techniques is an excellent option.

When there is only minor recession, some healthy gum tissue often remains and protects the tooth, so that no treatment other than modifying home care practices is necessary. However, when no firm gum tissue remains, this leaves the roots of the teeth and the underlying bone relatively unprotected, which could result in root sensitivity, bacterial penetration, decay and even loss of teeth.

A gingival graft is designed to address these problems. A thin piece of tissue is taken from the roof of the mouth or gently moved over from adjacent areas to provide a stable band of attached gum tissue around the tooth. The gingival graft may be placed in such a way as to cover the exposed portion of the root, however, the main objective is to re­establish the protective barrier or layer of the gum around the tooth. Occasionally, the patient may need multiple procedures to achieve the ideal amount of root coverage.
